How do a base policy and a super top-up split a hospital bill?
A base health policy pays the hospital bill up to its sum insured, subject to room rent, co-pay, and exclusions. A super top-up is a second policy that starts paying only after covered bills in that year cross a deductible you chose. It is useful when you want a large ceiling without paying a large-sum-insured premium on the base plan. It does not pay the first rupee, and an advertisement that blurs top-up and super top-up is hiding which bill has to be paid twice.
Deductible per stay, or deductible for the year
Policies sold as a top-up usually apply the deductible to each hospitalisation. Two admissions below that deductible can both go unpaid by the top-up even though the year’s bills are large. A super top-up usually adds those covered bills through the policy year and pays once the total crosses the deductible. Confirm the word “aggregate” or “per year” in the wording. The product name on a website is not the contract. Set the deductible at a number you can pay, or that a base policy will actually pay. A deductible of ₹10 lakh with a base policy of ₹5 lakh means you are uninsured for the gap.
What the top-up copy leaves out
The super top-up has its own waiting period, room-rent rule, and list of non-payables. A base policy that pays a claim does not force the top-up to pay the rest on the same terms. Match the room category you would take. If the base policy cuts the bill because you took a room above the cap, the unpaid slice may also sit outside the top-up. Buy both from a cashless network you would actually use, or accept that one of them may be a reimbursement. Pre-authorisation on one insurer is not pre-authorisation on the other. The sequence is in what cashless approval does and does not settle.
Who should skip the second policy
Skip a super top-up if the base sum insured already covers a realistic bill in your city and you will not fund the deductible. Skip it if the deductible is larger than the base policy, or if the top-up excludes the treatment the base policy is there for. Parents with an existing illness need that illness accepted on both contracts, or the second policy is a brochure.

Questions people ask next
Is employer cover the base policy?
Only while you are employed and the employer chooses to offer it. It can sit under a super top-up you buy yourself if the deductible matches. When you leave, the base disappears and the deductible is suddenly your cash unless a personal policy replaces it.
Do I file two claims for one admission?
Often yes, if both insurers are involved. The base insurer settles up to its limit. You then claim the remainder from the top-up with the discharge summary and the base settlement letter. Ask the hospital which policy they will swipe first.
Can the deductible be paid in cash?
Yes, if the wording allows it, and then you still meet the deductible with bills that the top-up accepts as covered. A bill for an excluded item does not count toward the deductible.
