What does a cashless approval actually settle?
A cashless approval means the insurer has told the hospital it will pay an estimate for a treatment at a network hospital, within your policy. It is not the final amount, and it is not a promise that the discharge bill will be zero. The number on the pre-authorisation can be revised when the bill arrives, and items the policy never covered were never going to be paid.
What happens between admission and discharge
Carry the policy, a photo identity, and the employee card if a corporate plan is involved. The hospital sends a pre-authorisation request with the treating doctor’s estimate. The insurer or the third-party administrator replies with an amount, a denial, or a query. While you are admitted, a longer stay or a procedure that was not on the estimate needs a fresh request. At discharge, the hospital sends the final bill. The insurer then pays what the contract allows and leaves the rest on your counterfoil. People are surprised here because the first message sounded like a yes. Pay the unpaid lines before you leave if the hospital asks. A reimbursement claim, if your policy allows it, is a separate filing for amounts you paid yourself. Denial of cashless is not always denial of the claim.
Lines that stay on your bill
Read these next to the approval message. Keep the discharge summary, investigation reports, and the insurer’s settlement letter. If you later claim a balance, that letter is the evidence of what was already paid.
- Room rent above the policy cap can shrink the whole bill, not just the room line, when the wording says so.
- Consumables, gloves, and toiletries are often on the non-payable list even when the surgery is covered.
- A co-pay is a percentage that remains yours after the insurer calculates its share.
- A waiting period that has not ended can remove the illness from this claim even if the hospital sent the file.
Who should not rely on cashless alone
If your hospital is outside the network, cashless will not be offered and you should know the reimbursement clock before admission. If the treatment is still inside a waiting period, an approval request wastes a day you could spend arranging funds. If two policies are in play, name which one is first. The second insurer will want the first insurer’s papers. See how a base policy and a super top-up split the bill.

Questions people ask next
The hospital says cashless was rejected. Is the claim dead?
Not always. Ask for the reason in writing. Some rejections are “not a network hospital” or “documents missing.” If the treatment is covered, you can pay and file for reimbursement within the time the policy states. If the reason is an exclusion or a waiting period, reimbursement will fail for the same reason.
Can I move to a single room if the policy says twin sharing?
You can ask. If you cross the room cap, the policy may cut more than the rent difference. Confirm that cut with the insurer before you shift, not at discharge.
Who do I chase if the payment is stuck?
The hospital chases the cashless desk during the stay. After discharge, your complaint is with the insurer and, if you named them, the third-party administrator. Note the pre-authorisation number, the time of each reply, and the rupee amount still unpaid.
